What does my monthly membership cover?

Our Direct Lifestyle Primary Care (DLPC) Membership includes a lot of benefits! For one low, monthly membership fee, you get access to all the care you want. Maybe you need a little extra support and help with a health issue, and would like to have weekly appointments for a bit? All included in your membership! Perhaps when you get that health issue under better control you want to have an appointment once per month to stay on track? It's included in your membership! Plant Based NP Lifestyle Care Center partners with a Laboratory Testing company, and your membership gets you access to laboratory testing for a small fraction of the price you would normally pay! Need some help learning to shop for, prepare, and cook healthy, delicious meals? It's included in your membership! Do you want to address the root cause of your health issues, and have the chance to reverse them and get off your medications? The support you need is included in your membership! We don't believe that patients should be penalized and burdened with extra costs, co-pays, coinsurance, hidden fees, and surprise medical bills for needing an appointment to address their health concerns with their provider. So at Plant Based NP, we simply include your care services in your membership; so we can focus on what's truly important - your health.

Will you be my Primary Care Provider (PCP)?

At Plant Based NP Lifestyle Care Center, we specialize in Lifestyle Care. Lifestyle Care does cover a lot of issues that make up Primary Care; but not all. While we do address a lot of the same chronic health conditions, preventative care, and wellness recommendations that a Primary Care Provider (PCP) would also offer; a PCP is better equipped to address other heath needs; such as acute illnesses and problems, vaccinations, etc. We believe that our patients are best served by continuing to see their PCP, along with us. The goal would be that we help our patients prevent, halt the progression of, and reverse chronic diseases; and improve their health and wellness - which would free up their PCP to address other acute and preventative issues. As our patients get healthier through this team approach, they will need to see both us, and their PCP, less and less!

Do you accept my insurance?

At Plant Based NP Lifestyle Care Center, we don't accept any insurance. We know that sounds strange, but it is actually a very good thing for both us, and our patients - and can end up being more cost effective for our patients in the long run. Insurance companies create weird, complicated rules. They decide the care you can and cannot get, based on whether they will pay for it. They create artificially increased prices for healthcare services. They make it very difficult for providers to spend an appropriate amount of time with their patients. The amount of money a patient is responsible to pay for healthcare services is often a complete mystery: the patient doesn't know, the provider doesn't know, and the insurance company can't tell you how much you will owe because the way they decide how much they will pay is a proprietary secret. In our current healthcare system, if they want to be compensated well, a provider has to put just as much effort into taking care of insurance companies as they do taking care of their patients; leading to higher costs and less face-to-face time with patients. At Plant Based NP, our goal is to provide incredible care to patients at an extremely affordable price; so we took insurance out of the picture! An entire month of care at Plant Based NP only costs about the same as what you would pay as a co-pay for just one specialist appointment using health insurance. We are sure that once you see what care directly between a provider and patient can be, you'll never want to go back to the traditional healthcare model!

Can I pay for my care with my FSA or HRA?

The services that Plant Based NP Lifestyle Care Center provides should be covered by both Flexible Spending Account (FSA) and Health Reimbursement Account (HRA) plans:

  1. Non-member Appointments can be paid for with an FSA and HRA.

  2. Laboratory testing, medications, etc. that are ordered by our clinic can be paid for with an FSA and HRA

  3. Direct Primary Care (DPC) Memberships should be eligible for FSA and HRA reimbursement.

Can I pay for my care with my HSA?

The answer to whether Health Savings Account (HSA) money can be used depends on a few factors:

  1. Non-member Appointments can be paid for with an HSA.

  2. Laboratory testing, medications, etc. that are ordered by our clinic can be paid for with an HSA.

  3. At this current time, due to an odd tax code technicality, a Direct Primary Care (DPC) Membership cannot be paid for with an HSA. However, there are currently several organizations and groups who are working hard to address and overturn this technicality.

Is there a fee to sign-up for a membership?

There is no fee to join Plant Based NP Lifestyle Care Center as a patient!

What if I decide to cancel my membership?

We will be sad to see you go, but you will never get charged any fees for terminating your membership.

However, if you do decide to return as a patient:

  1. We reserve the right to charge a Re-Enrollment Fee.

  2. If we are at our maximum number of patients, we may be unable to re-establish care with you right away; and you may need to be placed on a waitlist.

Are there any fees or costs outside of my membership?

There are a few instances in which a patient may be charged more than just their membership fee:

  1. If you pay for your membership via debit/credit card, you are responsible for paying the small service fee. However, we can assist you in setting up a different method that has no service fees.

  2. The cost of laboratory tests are not included in the membership, however they are extremely discounted, and we will always tell you what a test costs before we order it. We can also order laboratory testing through your insurance company; but it may still be cheaper to pay our discounted price.

  3. There may be some future events or services that cost a small, reasonable amount of money on top of your membership (for example, $5 for a group yoga session). However, these will always be optional; and we will always be fully transparent in telling you any additional costs beforehand.

Do I need to make an appointment?

You do need to make an appointment; we do not have any walk-in availability. However, we typically are able to see you very quickly; sometimes even the same day you call!

Do I have to live in Columbus, OH to be a patient?

No! While our office is in Columbus, we can take care of anyone in the State of Ohio! We offer in person visits if you live nearby or are visiting Columbus. We also offer virtual Telehealth visits regardless of your proximity to Columbus!

Can I be a patient if I live outside of Ohio?

Technically, yes! But there is a catch: We can only take care of patients that are physically present in the State of Ohio. If you are coming to visit Ohio and would like an appointment, we are happy to see you. If you live on the border of Ohio and another state, and you drive into Ohio for your appointment, we are happy to provide care to you!

If you don't live in Ohio, and can't visit, we can't offer Nurse Practitioner services to you. However, if you are really interested, there is a chance that we can provide you with Health and Wellness Coaching - depending on your home-state laws. In this instance, we can provide general information about Lifestyle Care principles, and support in improving health. However, we cannot provide any personalized, person-specific care through this coaching; and will be operating solely as a Health and Wellness Coach; not a healthcare professional.

What methods of payment do you accept?

We accept multiple methods of payment for your convenience. Some electronic methods include processing fees that will be charged to you. We can work with you to help you choose, and use, a method that does not have any associated processing fees.

Electronic Methods:

  • CashApp (No Processing Fee) - We can help you set this up if you are unfamiliar with CashApp.

  • ACH Transfer (1% Processing Fee)

  • Credit/Debit Card (2.9% + 30¢ [3.4% for American Express] Processing Fee)

Other Methods:

  • Cash

  • Check (for patients that we know and trust)

What do all the letters after Marty's name mean?

MSN = Master of Science in Nursing. Marty has completed both a Masters Degree and Post-Masters Degree in Nursing. He is also currently in the process of pursuing his Doctorate in Nursing Practice (DNP).

APRN = Advanced Practice Registered Nurse. This is a designation by the State of Ohio that Marty has met the requirements, and is licensed to practice in an advanced practice nursing role.

CNP = Certified Nurse Practitioner. This is a designation by the State of Ohio that Marty is licensed to practice, treat, and prescribe as a Nurse Practitioner.

FNP-C = Certified Family Nurse Practitioner. This is a designation by the American Association of Nurse Practitioners Certification Board that Marty has met the requirements for, passed the certification exam; and is certified to provide primary care services to patients across the entire lifespan (pediatric, adolescent, adult, pregnant, elderly, etc.)

CRNA = Certified Registered Nurse Anesthetist. In addition to being a Nurse Practitioner, Marty is also a Nurse Anesthetist. A CRNA is an Advanced Practice Nurse who provides a broad range of anesthesia care services to patients. While he does not use this certification in his role at Plant Based NP, it is a testament to his advanced clinical knowledge and professional acumen.
